Browsers: You can access the service, obtain forms and save them to a PC or iPad using most browsers.

PDF Viewer: To fill in and save any interactive form that has been obtained from the service, you must use one of the following PDF viewers:

  • Version XI or DC of Adobe Acrobat Reader on a PC. Version DC (which is the latest version) may be downloaded free of charge from https://get.adobe.com/uk/reader/)
  • Adobe Acrobat on a PC (version 9 or later recommended)
  • Readdle PDF Expert app on an iPad (an app available from the Apple Store for just a few pounds)

You may use earlier versions of Adobe Reader (version 9 or X recommended) to fill in the form and to print it. However, they have a restriction such that when you save the form, the data that you have entered will not be saved. If you need to save and/or email the completed form, it is important, therefore, to use version XI or later.

Registration and Credits: To use the service, you need to register. You may also need to purchase or be donated credits. However, once you have registered, you will immediately be able to download, experiment with and use the self assessment questionnaire; no credits are required for this. Credits may be transferred (or donated) between registered users so, for instance, a designated body or a hospital trust may acquire a batch of credits to transfer to a group of hospitals or doctors’ practices.
